The Native Aging in Place Project: Building Sustainable LTSS Capacity in your Tribal Community
The Native Aging in Place Project (NAPP) works to create a sustainable infrastructure of long-term services and supports for the Spirit Lake Tribal elders by creating and implementing best practices that allow Native American Elders to live independently in their homes. In this webinar, the presenters will discuss the NAPP successes and lessons learned. They will share best practices in strengthening local capacity to deliver direct services to the elders and how the NAPP assisted in developing a sustainable model for community-based services. The information shared in this presentation will provide insight on the Spirit Lake Tribe’s model in building a skilled provider agency in the community to provide direct services to the elders.

Learning objectives:
• Discuss the value of the Native Aging in Place Project.
• Learn how to accelerate and strengthen local capacity to develop, deliver, and evaluate a Long-Term Services & Supports program for your Tribal Elders.
• Discuss how to develop a sustainable model of community-based services to assist Tribal Elders to age in place in their community.
